Mitchel Erickson Presents On Older Adult Communication and Care

Dr. Mitchel Erickson presented content relevant in Geriatric clinical care settings at two conferences. The first conference was an opportunity to bridge the gap between transitions of care between long term and rehabilitation facilities in California and Emergency Departments. Older adults are particularly vulnerable to transitions of care and improving communication and expectations during the transitions is imperative for healthcare improvement. The second conference presented collaborative content from all five UC Medical Centers in California as they expand their Age-Friendly Health System approaches to older adult care in emergency departments. Unidentified delirium and cognitive changes have the potential to extend lengths of stay and discharge patients to situations that create a cycle of crisis-related return to care. Best practice approaches were shared.
