CEC Was Awarded Grant To Improve Health Literacy In Underserved Communities

In 2022 the Charles Evans Center was awarded a $200,000 grant from the Mother Cabrini Foundation to improve health literacy in the Nassau County area by providing free health screenings, workshops on chronic health conditions, and by facilitating referrals to care.

The goal of the grant is to eliminate barriers to care for medically underserved communities, and to improve health literacy with education, tools and resources. Community outreach will help address the unmet need for no- and low-cost services that target prevention and education among underserved communities. Providing opportunities for community members to engage with health professionals will provide an avenue for the underserved to obtain culturally appropriate health information and health screenings with the goal of reducing health disparities.

Through the grant the Charles Evans Center is hiring a two-person outreach team consisting of a Care Navigator and a Licensed Practical Nurse (LPN). The team will attend health and wellness fairs in Nassau County where individuals can receive free health screenings, health education and connections to resources in a relaxed atmosphere. The team will also seek to partner with local government, schools, religious organizations and other social service agencies to offer health classes and workshops on various health topics to include: Diabetes, Cardiovascular Disease, Maintaining Wellness, and Health Aging. In response to the Opioid and Fentanyl crisis, the team will also seek to educate youth and their families about those substances.
