We Want to Hear From You! Take the PD Active Survey

PD Active’s strength lies in the fact that our programs and priorities are set "by and for" those affected by Parkinson’s disease in the Bay Area.

We invite you to complete our annual survey and tell us what you value about PD Active, what programs, classes and events are most important to you, and additional resources that you would like to see PD Active offer.

Your opinions matter.

The Board of Directors will use the results of the survey in our planning and budgeting going forward.

The survey only takes about 10 minutes — please fill it out today!
